29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    a) On May 15, 2024, employees were exposed to eye injury while operating pneumatic nail guns without the benefit of eye protection.  Hernandez Reyes Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 1725234, citation 1, item 1, and was affirmed as a final order on April 16, 2024, with respect to a worksite located at Lot 62-21127 Preserve Dr., Hawthorne Woods, Illinois.  No abatement certification or documentation required for this violation.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) On May 15, 2024, an employee was performing framing carpentry work without utilizing fall protection while exposed to fall hazards of approximately 13.2 feet.  Hernandez Reyes Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1725234, citation 2, item 1, and was affirmed as a final order on April 16, 2024, with respect to a worksite located at Lot 62-21127 Preserve Dr., Hawthorne Woods, Illinois.  Hernandez Reyes Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1701219, citation 1, item 1, and was affirmed as a final order on November 1, 2023, with respect to a worksite located at 331-337 Lasalle Lane, Buffalo Grove, Illinois.  No abatement certification or documentation required for this violation.
