29 CFR  1910.132(d)(1):  The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Production Floor:  On, or about, May 21, 2024, the employer had not conducted a Hazard Assessment to determine what personal protective equipment was required for employees who operated printing equipment and who performed maintenance, set-up and cleaning tasks on and around printing equipment.

29 CFR  1910.133(a)(1):  Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  Production Floor:  On, or about, May 21, 2024, employees who used press cleaning chemicals such as, but not limited to, Gotham Ink Water Works Cleaner 21 and 18-M Cleaner & MRC, both of which could cause eye damage, were not provided with protective eyewear.  Also, employees did not wear eye protection while operating and maintaining printing equipment.

29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Production Floor:  On, or about, May 15, 2024, the employer had not developed and implemented a Lockout/Tagout program to protect employees during servicing and maintenance tasks for machinery and equipment in the plant.    Abatement Note:  Tasks other than normal production operations such as clearing jams and machine adjustments are covered under the scope of the lockout/tagout regulation but may be accomplished with alternative procedures depending on the press control system and the type of safeguarding provided on the press.
