29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about May 23, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On or about May 23, 2024, at the above addressed jobsite, employees performing commercial carpentry framing operations, approximately twenty (20) feet above a lower level, were not protected from falls, thereby exposing the employees to fall hazards.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a.  On or about May 23, 2024, at the above addressed jobsite, the ladder used to access and egress the working surface was a ten-foot A-Frame ladder that was closed and leaned against the structure, thereby exposing the employees to fall hazards.
