29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New single-family residential structure located on Lot #116 at 5601 Overview Dr., Flowery Branch, GA 30542, Ground and scaffold: On or about 02/10/2025, the employer exposed employees to struck-by hazards while operating circular saws and pneumatic nail guns in that protective eyewear was not provided while cutting and installing siding material.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  a) New single-family residential structure located at Lot #116 at 5601 Overview Dr., Flowery Branch, GA 30542, Ground level: On or about 2/10/2025, the employer exposed employees to amputation hazards while operating  Makitia 5007F 7-1/4-inch circular saws, in that the lower guard did not retract immediately after a cut was made as a chock was used to keep the lower guard in the open position.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) New single-family residential structure located at Lot #116 at 5601 Overview Dr., Flowery Branch, GA 30542, Scaffold Area: Employees were exposed to fall hazards of approximately 15 feet 6 inches to the lower ground below while utilizing a pump jack scaffolding system, in that a fall protection system was not available or utilized.
