29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about May 20, 2024, at the above addressed jobsite, the portable extension ladder used to access the roof did not extend at least three (3) feet above the roof's surface, thereby exposing the employees to fall hazards.

On or about May 20, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.
