29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Located at 18224 45th Place, Denver, CO, 80249: On or about May 17, 2024 and at times prior, the employer did not ensure employees were provided eye protection while operating pneumatic nail guns while installing roof shingles. This condition exposed employees to projectile hazards to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 18224 45th Place, Denver, CO, 80249: On or about May 17, 2024 and at times prior, the employer did not ensure that employees were protected from fall hazards greater than 6 feet in that the employees were not tied off while performing roofing operations on a single-family multi-story residential home. LEMT Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1700737, citation number 1, item number 1 a and was affirmed as a final order on (11/17/2023), with respect to a workplace located at 13693 W. Amherst Place, Lakewood, CO, 80228.
