29 CFR  1910.95(g)(5)(i): Within 6 months of an employee's first exposure at or above the action level, the employer shall establish a valid baseline audiogram against which subsequent audiograms can be compared.  On or about August 6, 2024, at the establishment located at 111 North Douglas Street, Hortonville, Wisconsin; the employer did not ensure that a baseline audiogram was obtained for each employee with noise exposure that exceeded 82.1 decibels measured on an A-weighted scale (dBA) in the following instances:  a) On or about August 6, 2024, an Assistant Operator employee was exposed to a 12-hour time-weighted average (TWA) sound level of 92.3 dBA, or equivalently, a noise dose of 137.9%. This exceeds a 12-hour TWA action level (AL) of 82.1 dBA, or equivalently, a noise dose of 50%. The sampling was performed for 667 minutes. b) On or about August 6, 2024, an Operator employee operating was exposed to a 12-hour TWA sound level of 91.3 dBA, or equivalently, a noise dose of 121.2%. This exceeds a 12-hour TWA AL of 82.1 dBA, or equivalently, a noise dose of 50%. The sampling was performed for 667 minutes.

29 CFR  1910.95(k)(2): The training program shall be repeated annually for each employee included in the hearing conservation program. Information provided in the training program shall be updated to be consistent with changes in protective equipment and work processes.
