29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) On or before 6/1/2024, the employer did not ensure that employees decking the second floor of a two-story single-family home under construction wore safety glasses. Employees were using pneumatic nail guns to nail down the plywood decking. This condition exposed employees to flying objects and nails.

Employees were decking the second floor of a new two-story single-family home without wearing fall protection. This condition exposed employees to a fall hazard in excess of 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or before 6/1/2024, the employer did not ensure that employees set up and used the extension ladder used to access the second story in a safe manner. The extension ladder did not extend at least three feet past the edge of the second story and did not extend at all past the edge. This condition exposed employees to a fall hazard in excess of 10 feet.
