29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: a) Mill Room - On or about 8/29/2024, an employee working near the extruder machine was exposed to continuous noise dose of 117.4%, or an equivalent of 91.2 dBA that exceed an 8-hour time-weighted average of 85 dBA. The employer failed to ensure that a monitoring program was developed and implemented to employees exposed to noise levels at or above action level of an 8-hour Time Weighted Average (TWA) of 85 dBA. The sampling was performed for 435 minutes during a working shift on 8/29/2024 when exposure calculations included a zero increment for the 45 minutes not sampled.
