29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i).  On or about April 18, 2024, and at times prior thereto, an employee was exposed to a fall hazard greater than 4 feet while repairing the siding on the rooftop of an existing building without fall protection.

29 CFR 1910.28(b)(12)(i): The employer did not ensure that each employee on a scaffold is protected from falling in accordance 29 CFR part 1926, subpart L.  On or about April 18, 2024, and at times prior thereto, employees were performing siding repairs while standing on a pallet that was raised by a forklift higher than 10 feet off the ground, exposing the employees to a fall hazard.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.  On or about January 24, 2024, and at times prior thereto, an employee was exposed to a fall hazard greater than 4 feet while inside a metal scaffold platform being lifted by a Powered Industrial Vehicle (forklift) without receiving approval from the forklift manufacturer to attach the platform.
