Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On or about June 4, 2024, employees of VK Cifuentes Contractors LLC were performing roofing work at 5300 Butler Hill Road in Saint Louis, Missouri. The employer did not ensure that fall protection was worn while work was being performed on the roof approximately 25 feet above the lower level. The employees were exposed to fall hazards to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: On or about June 4, 2024, employees of VK Cifuentes Contractors LLC were performing roofing work at 5300 Butler Hill Road in Saint Louis, Missouri. The employer did not ensure an extension ladder placed on the East side of the building was properly positioned. The ladder side rails did not extend three feet past the upper landing surface, exposing employees to fall hazards.
