29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet or more above a lower level is protected from falling by guard rail systems, safety net systems, or a personal fall protection system: On or about June 5, 2024, maintenance employees engaged in grain handling activities at the Rail Modal Group facility located at 549 East County Road T, Fremont, Nebraska, were exposed to fall hazards. The employees were observed standing and working from the top of an approximately 8-foot-tall storage container without utilizing any form of fall protection.
