29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  On or about November 29, 2024, it was documented that employees engaged in wheel servicing activities in the maintenance department of Rail Modal Group, LLC located at 549 East County Road T, Fremont, Nebraska were exposed to respiratory hazards in that employees who wore N-95 masks to protect against particles and fumes generated during grinding and welding on mild steel were not provided the information included in Appendix D of �1910.134.

29 CFR 1910.177(c)(2): The employer did not assure that each employee demonstrated his ability to safely service rim wheels by effective performance of all tasks listed in 29 CFR 1910.177(c)(2)(i) through (viii):  On or about September 24, 2024, it was documented that employees engaged in wheel servicing activities in the maintenance building of Rail Modal Group LLC located at 549 East County Road T, Fremont, Nebraska were exposed to struck-by hazards in that the employer did ensure employees' ability to safely service single-piece rim wheels was demonstrated prior to performing such service.  Instances included all tasks as required by and listed in ��1910.177(c)(2)(i) - (viii).

29 CFR 1910.177(d)(2): The employer did not provide a restraining device or a barrier to be used when inflating tires on single piece rim wheels which are not bolted onto the vehicles:   On or about September 24, 2024, it was documented that employees engaged in wheel servicing activities in the maintenance building of Rail Modal Group LLC located at 549 East County Road T, Fremont, Nebraska were exposed to struck-by hazards in that the employees inflated 255 and 295 R275 tires on single-piece semi-truck rim wheels either while freestanding upright on the tire tread or laid flat on the ground.
