29 CFR 1910.106(e)(6)(i):Adequate precautions were not taken to prevent the ignition of flammable vapors: a) Selective Coatings & Inks, Inc. - Front room/Dispensing area: A grounding wire was not used where employees transferred flammable materials from full to empty drums. On 06/05/2024 an employee was transferring dimethyl carbonate, a flammable chemical, into an empty drum without a grounding wire and the drum exploded.

29 CFR 1910.106(h)(4)(iv)(a): Equipment was not designed and arranged to prevent the unintentional escape of liquids and vapors and to minimize the quantity escaping in the event of accidental release. a) Selective Coatings & Inks, Inc. - Front room/Dispensing area: The Flux Pump - Model # F416/1 - S/N 41614324, used to transfer flammable liquids from one drum to another, was not designed or configured to prevent unintentional escape of liquids and vapors. On 06/05/2024 the pump continued to operate after a drum exploded resulting in the release of flammable liquid into the work area.

29 CFR 1910.132(a): Protective equipment was not used when necessary, whenever hazards capable of causing injury and impairment were encountered: a) Selective Coatings & Inks, Inc. - Front room/Dispensing area: The employer did not enforce the use of flame-resistant uniforms, including long-sleeve shirts and long pants, to protect employees from burns when transferring and handling flammable chemicals. On 06/05/2024 an employee sustained second degree burns after the polyester clothing he was wearing caught fire due to an explosion.
