On or about June 10, 2024, in the facility, the employer failed to ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections were established for employees performing service and/or maintenance on multi-energy source equipment, but not limited to, CNC boring mill machines. Employees were thereby exposed to machine hazards associated with unexpected energization of equipment
