29 CFR  1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a.  On or about June 10, 2024, in the main furnace, contractors rebuilding a furnace were exposed to falls of approximately eight-nine inches (89") when working at the edge of the furnace, an unprotected side.

29 CFR  1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine it was safe for entry:     a.  On or about June 3, 2024, in the facility, employees were exposed to hazards associated with permit required confined spaces when the employer failed to verify that conditions inside the space were acceptable for entry.  Potential employee exposure included a hazardous atmosphere from chlorine and nitrogen gas(es) after the employer failed to isolate the space and evaluate the atmosphere prior to first entry.

29 CFR  1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:    a.  On or about June 3, 2024, at the jobsite, the employer failed to complete a permit required confined space entry permit for employees entering the furnace to perform furnace relining activities.                   Employees within the space were, thereby exposed to hazards associated with entry into a permit required confined space.

On or about June 3, 2024, at the jobsite, employees working in the main furnace were exposed to hazards associated with permit required confined space(s) when the employer failed to document through a written certification the measures taken to prepare the space for safe entry and that all hazards in the space were eliminated prior to entry.
