On or about May 28, 2024, Schad Boiler Setting Company dba Schad Refractory Construction Company (Schad) failed to ensure that Schad and the host employer, Trialco Aluminum, LLC, informed each other of their respective lockout and tagout procedures prior to performing refractory relining activities on the SF Reverberatory Furnace.  As a result, employees were exposed to the potential for an unexpected release of atmospheric gas from the chlorine and nitrogen gas line that was present inside of the furnace and controlled by the molten metal pump.

Affected employees, thereby were exposed to hazards during the relining activities to the SF Furnace from the presence of natural and nitrogen gas lines located within the furnace.
