29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached: 10 Coachman Ridge Rd., Shrewsbury, MA: On or about June 11, 2024, an employee performing roofing work was attached to an anchor point that was installed incorrectly, without the required number and specific type of attachment fasteners required for proper installation.
