29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 10, 2024, employees were using an extension ladder to access West side of the roof at the site listed and the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed.

29 CFR  1926.1053(b)(16):Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired:  On or about June 10, 2024, employees were using a section of an extension ladder to access an upper landing surface on the West side of the roof at the site listed that had a broken rung.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about June 10, 2024, five employees engaged in roofing activities to include installing rolled rubber roofing and other roofing materials on a low-slope roof with unprotected sides and edges as much as 17' above lower levels, were not protected from falling by any acceptable means.   City Professional Restoration, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1659405, citation number 1, item number 2 and was affirmed as a final order on 7-20-2023, with respect to a workplace located at 1785 S Eastwood Dr., Woodstock, IL 60098.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 10, 2024, employees were exposed to fall hazards of approximately 17' while performing commercial roofing work. The employer did not provide a training program to recognize fall hazards and the procedure to be followed in order to minimize those hazards.  City Professional Restoration Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1659405, citation number 1, item number 3 and was affirmed as a final order on 7-20-2023, with respect to a workplace located at 1785 S Eastwood Dr., Woodstock, IL 60098.
