29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about September 20, 2024, at a jobsite located at 7013 143rd Ct., Kenosha, WI 53140, employees working on the ground performing cleanup and assisting the roofers who were working directly above roofing the structure, were not protected from struck by hazards from falling materials and tools by wearing a protective helmet.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about September 20, 2024,  at a jobsite located at 7013 143rd Ct., Kenosha, WI 53140, employees conducting roofing activities (paper and shingle installation) on a 10:12 pitch roof of the new construction residential structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the first floor roof of approximately 12 feet on the North and West sides of the structure, and 25 feet from the second floor roof on the North, South, and West sides of the structure.  City Professional Restoration, Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1659405, citation number 1, item number 2 and was affirmed as a final order on 7-20-2023, with respect to a workplace located at 1785 S Eastwood Dr., Woodstock, IL 60098.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about September 20, 2024, at a jobsite located at 7013 143rd Ct. Kenosha, WI 53140, the employer did not ensure that employees exposed to potential fall hazards of 12 feet or greater, while working from a 10:12 pitch roof had been properly trained to recognize, minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard onto dirt, building materials, and  gravel.  City Professional Restoration Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1659405, citation number 1, item number 3 and was affirmed as a final order on 7/20/2023, with respect to a workplace located at 1785 S Eastwood Dr., Woodstock, IL 60098.
