On or about June 7, 2024, in the HFC-245fa Process Unit, the employer did not effectively implement procedures for properly implementing safe and effective lock-out/ tag-out and line opening, specifically block-bleed blind flange side of a closed bleed valve on the D reactor fresh feed system (Valve F105).

29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained, or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices. On or around June 7, 2024, on the fifth floor of Hydrofluoric Acid Unit 245 at Honeywell Geismar, the employer did not ensure that all potentially hazardous stored or residual energy was properly bled, disconnected, blocked, or otherwise made safe, specifically the block-bleed blind flange side of a closed bleed valve on the D reactor fresh feed system (Valve F105).
