Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. a) On or about June 4, 2024, at 124 East Bronson Road in Seymour, Wisconsin, in Boxing Room Packaging, employees were exposed to potential burn hazards while picking up cups from the floor and sanitizing machines and floor between a wall and the backside of the Cup Line without the work area access at least 28 inches wide. b) On or about June 4, 2024, at 124 East Bronson Road in Seymour, Wisconsin, in Boxing Room Packaging, employees were exposed to potential burn hazards while performing machine maintenance activities between a wall and the backside of the Cup Line without the work area access at least 28 inches wide.

The width of working space in front of the electric equipment shall be the width of the equipment or 762 mm (30 in.), whichever is greater. In all cases, the working space shall permit at least a 90-degree opening of equipment doors or hinged panels. On or about June 4, 2024, at 124 East Bronson Road in Seymour, Wisconsin, in Boxing Room Packaging, the employer did not ensure a safe means to access and for workspace in front of the labeler machine electric panel located on the Cup Line. The working area in front of the panel door was 20.5 inches deep.

29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving. On or about June 4, 2024, at 124 East Bronson Road in Seymour, Wisconsin, in Boxing Room Packaging, employees were exposed to struck-by and crushing hazards during operating of the Cup Line without the labeler machine anchored to the floor. Employees working at sides and under the machine during operation picking up cups from the floor where exposed to struck-by and crushing hazards caused by the machine tipping over.
