29 CFR 1910.176(a):Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a) On or before 6/12/2024 the employer did not ensure that when dock doors such as dock door "D" were opened for fresh air that they were blocked to prevent forklift operators from driving off the 47" dock where mechanical handling equipment such as the Crown forklift (SN# 10332278) were being utilized to load totes 3 tier high of parts into trailers. During 3rd shift, an employee operating the Crown forklift was turning around after driving backward with the high load that was difficult to see around to turn into dock "C" and drove off of dock "D" with the load, there was not sufficient safe clearance for the operator to turn safety into dock "C" such as having dock door "D" closed.
