29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure(s), employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) ASB Sports Acquisitions Inc on or about June 14, 2024, at the address of 11500 Tailwinds Dr. Swanton, OH: Maintenance employees tasked with lubricating the carousel shaft on equipment such as but not limited to printing presses, without the benefit of energy isolation program(s), procedure(s), devices and training, were exposed to rotating and pinch point hazards. b) ASB Sports Acquisitions Inc on or about June 20, 2024, at the address of 11500 Tailwinds Dr. Swanton, OH: Operators and operator's assistants tasked with setup activities of the printing presses without the benefit of energy isolation program(s), procedure(s), devices and training were exposed to struck-by hazard from the unexpected energization of the machine's carousel.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) ASB Sports Acquisitions Inc on or about June 20, 2024, at the address of 11500 Tailwinds Dr. Swanton, OH: The employer did not ensure an effective means of guarding was implemented on the M&R Sportsman EX screen printing press. Employees tasked with working from "operator & assistant" workstations were exposed to struck by and caught in hazards of the machine's indexing pallet arms attached to the rotating carousel. b) ASB Sports Acquisitions Inc on or about October 24, 2024, at the address of 11500 Tailwinds Dr. Swanton, OH: The employer did not ensure an effective means of guarding was implemented on the M&R 2000 dryer #6 discharge conveyor. Employees tasked with removing garments from the conveyor were exposed to incoming nip points created between the belt and drive roller.
