29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  ASB Sports Acquisitions doing business as Game One located at 11500 Tailwinds Drive in Swanton, Ohio;  On or about October 17, 2024 the employer did not ensure employes wore eye and face protection when exposed to chemical acids. Employees working in the Screen Reclaim Department handling cleaning chemicals such as, but not limited to, RISE Ink Wash Concentrate, Pregasol CF-20, and GEM-ZYME Concentrated Emulsion Remover, were exposed to hazards associated with sulfuric acid, sodium periodate, and orthoperiodic acid such as but not limited to skin irritation, eye irritation, chemical burns, and damage to organs through prolonged or repeated exposure.

29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  ASB Sports Acquisitions doing business as Game One located at 11500 Tailwinds Drive in Swanton, Ohio;  On or about October 17, 2024 the employer the employer did not establish and implement a written respiratory protection program with worksite specific procedures outlining (1) the selection of respirators for use, (2) medical evaluations of employees, (3) fit testing procedures, (4) cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators, (5) Training of employees in the respiratory hazards to which they are potentially exposed, (6) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance, and (7) procedures for regularly evaluating the effectiveness of the program. Employees were required to wear at a minimum 3M Particulate P95 Respirators when dispensing and spraying cleaning chemicals such as, but not limited to, RISE Ink Wash Concentrate, Pregasol CF-20, and GEM ZYME. Employees handling cleaning chemicals were exposed to hazards associated with sulfuric acid, sodium periodate, and orthoperiodic acid such as but not limited to skin irritation, eye irritation, chemical burns, damage to organs through prolonged or repeated exposure.

Employees were exposed to hazards associated with wearing a tightfitting respirator such as, but not limited to, aggravation of pulmonary issues, such as asthma, emphysema, and aggravate claustrophobia, all contributing to negative health effects.
