29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Maintenance Shop: On or about June 20th, 2024, an annual audit and/or review of the agency's lockout tagout program was not completed as required. Specific (written) machine procedures were not developed, and enforced by the employer. Employees tasked with and/or assigned to completed service and or maintenance at the facility were not protected from hazards associated with energized parts.
