29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) On or about May 22, 2024, employees were exposed to injuries from falls, while conducting reroofing activities, with the fall protection system lanyards being attached to the front of the wearers body harness.
