29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) On or about November 21, 2024, the exposing employer, DD Roofing LLC, did not ensure employees were protected from fall hazards in excess of 23 feet while engaged in residential re-roofing operations. The employees were not properly protected by a personal fall arrest system as all the components of that system were not connected; the employees did not have their ropes connected.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about November 21, 2024, the exposing employer, DD Roofing LLC, did not ensure the rails of the portable ladder extended at least 3 feet beyond the upper landing of the residential roof. The ladder was placed on the first story of the roof, and employees engaged in residential re-roofing operations were using it to access the second story of the 23-foot-high roof. This condition exposed employees to fall hazards.
