29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  On or about November 21, 2024, the exposing employer, DD Roofing LLC, did not ensure the rails of the portable ladder extended at least 3 feet beyond the upper landing of the residential roof.   The ladder was placed on the first story of the roof, and employees engaged in residential re-roofing operations were using it to access the second story of the 23-foot-high roof.  This condition exposed employees to fall hazards.
