29 CFR 1910.1030(c)(1)(i):Each employer having an employee(s) with occupational exposure as defined by paragraph (b) of this section shall establish a written Exposure Control Plan designed to eliminate or minimize employee exposure. On or about June 25, 2024, the employer did not establish a written exposure control plan when employees cleaned blood after an incident occurred.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. The employer did not report an employee hospitalization within 24 hours of being aware of the hospitalization. On or about June 5, 2024, an employee was hospitalized after their foot was caught under a forklift.
