29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees performing equipment maintenance and/servicing activities in Building A of Metalworks Inc. located at 301 P. Street, Lincoln, Nebraska were exposed to caught-in and amputation hazards in that the employer either did not develop or ensure employees used procedures to isolate hazardous energy. Instances included, but were not limited to: i. On or about June 27, 2024, employee(s) performed maintenance and servicing on the ByTrans 3015 Extended material loader chains and chain guide(s) without appropriate hazardous energy control procedures and ii. On or about July 8, 2024, employees entered Bystronic BySpirit Fiber 3015 laser(s) to clean while equipment was still energized.

Street, Lincoln, Nebraska were exposed to caught-in and amputation hazards in that the employer did not ensure employees applied lockout and/or tag out to relieve, disconnect, restrain, and/or otherwise render safe all potential hazardous stored or residual energy. Instances included, but were not limited to: i. On or about June 27, 2024, employee(s) performed maintenance and servicing on the ByTrans 3015 Extended material loader chains and chain guide(s) without appropriate hazardous energy control procedures and ii. On or about July 8, 2024, employees entered Bystronic BySpirit Fiber 3015 laser(s) to clean while equipment was still energized.
