29 CFR 1926.502(i)(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.  Site: 45-36 21st Street, Long Island City, NY 11101. On or about June 25, 2024, employees engaged in stripping aluminum precast concrete panel from dry concrete floors, and cleaning construction debris were exposed to two instances of floor holes opening that were not secured.  a)  5th floor of Building B, floor hole measured approximately 6 inch wide and 12 inch long. b) 5th floor of  Building A measured 16 inch wide and 27 inch long.   These hazards exposed employees to trip and fall hazard.   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
