29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  At an establishment 204 South Street, in Bonduel, Wisconsin,  (a)  On or about June 14, 2024, in the Fabrication Area, the established energy control procedures for the Caddy Fab 003 for the Niagara Brake press HMB-100-10-12 S/N: P53039 did not identify specific procedural steps for the verifying the de-energization of the hydraulic energy source as well as the use of die blocks during die changing, servicing, and maintenance activities exposing employees to struck by and caught between hazards.    (b)  On or about June 14, 2024, in the Fabrication Area, the established energy control procedures for the Coil Line 005 for the Punch Press Stamtec G1-80, S/N:  0399 did not identify specific procedural steps for the verifying the de-energization of the hydraulic energy source as well as the use of die blocks during die changing, servicing, and maintenance activities exposing employees to struck by and caught between hazards.

29 CFR 1910.147(c)(7)(i)(A):  Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about June 14, 2024 and thereafter, at an establishment 204 South Street, in Bonduel, Wisconsin, employees trained as authorized were not trained in the methods and means necessary to isolate the hydraulic energy source and use of die blocks while performing die changing activities on equipment such as but not limited to the Niagara hydraulic press brake and Stamtec hydraulic punch press.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  At an establishment 204 South Street, in Bonduel, Wisconsin,  (a)  On or about the week of June 14, 2024 and thereafter, employees were exposed to machine hazards associated with moving parts when they engaged in die changing activities on the Niagara Brake press HMB-100-10-12 S/N: P53039.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to set-up.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  (b)  On or about June 25, 2024, employees were exposed to machine hazards associated with moving parts when they engaged in die changing activities on the Stamtec hydraulic punch press.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to set-up.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.   (c)  On or about June 26, 2024, employees were exposed to machine hazards associated with moving parts when they engaged in unjamming activities on the Kent Corporation Burr Master, Model:  BM2-120 BC, S/N:  10767.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to unjam.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At an establishment 204 South Street, in Bonduel, Wisconsin,  (a)  On or about June 14, 2024, in the Fabrication Area, the Niagara hydraulic press brake did not have guarding between the part and frame while performing bending activities on small parts exposing employees to caught in between hazards.   (b)  On or about June 26, 2024, in the HPL Area, the glue line machine did not have guarding between the Southworth scissors lift conveyor and fixed conveyor exposing employees to caught in between hazards when the scissors lift lowered to the lowest position.  (c)  On or about June 26, 2024, in the HPL Area, the glue line machine had a singular safety mat used to protect employees against struck by hazards from the Midwest Automation PF251 board pusher.  The safety mat was not large enough to prevent employees from stepping over and being in the struck by area.    (d)  On or about June 14, 2024, in the Fabrication Area, the Accurshear 625010 shear did not have the hold down guarding adjusted when performing shearing activities exposing employees to caught in between hazards.     (e)  On or about June 26, 2024, in the Plastic Cutting Area, the Maggi Engineering Junior 640 Radial Arm Saw did not return to the original position upon release exposing employees to struck by hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  On or about June 20, 2024, at an establishment 204 South Street, in Bonduel, Wisconsin, in the Cut Room, the power cord for the Scotchman Radial Cut Saw motor was pulled from the motor housing exposing employees to electrical shock hazards.
