29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: VEHICLE BAY - On June 26, 2024, and at time prior to, a four-conductor flexible cord with plug receptacles was powering a satellite communication trailer. The service side end of the cord had a damaged outer insulative jacket revealing the inner conductors, exposing employees to electrical and fire hazards.

29 CFR 1910.334(a)(3)(ii):Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: VEHICLE BAY - On June 26, 2024, and at time prior to, a fan was located in the vehicle bay that did not possess a grounding plug, exposing employees to electrical hazards. The fan did not have an out of service tag that was possessed on other equipment.
