29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy source isolating device by authorized employees: A) 555 Lehigh Ave Union, NJ 07083 Production room Employees did not affix lockout or tagout devices to isolate hazardous energy sources to the Dima continuous mixing machine (Cooker/Stretcher 3) that included, but was not limited to the main power disconnect and heat source from steam generation. Employees are not required to lockout machine, while employees performed cleaning activities. The activity that was being performed at the time of the accident was to clear excess cheese from the cooker. Hazard occurred on or about 06/26/24
