29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed: a.) Warehouse - On or about June 26, 2024, a forklift operator driving Toyota Forklift, Model 8FBCH525, Serial No. 66784, failed slow down while where the employee's vision was obstructed by stacked product at a cross aisle. The employer failed to ensure forklift operators were required to slow down wherever vision was obstructed, thereby exposing employees to struck-by hazards.
