OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential flying objects:   Buildng 22, Sphericals:  On, or about, June 26, 2024, a stand mounted JET off-hand grinder was provided with a Type 1 straight abrasive wheel on the left side. The abrasive wheel was primarily used to sharpen bronze tools and the sharpening was done using both the periphery and side of the wheel.  The Type 1 wheel was designed for grinding on the periphery of the wheel, not the side of the wheel.

29 CFR  1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts, pinch points and flying chips:  On, or about, June 26, 2024, in the following locations, adequate machine safeguarding was not provided:  A.	Building 22, Bushings Value Stream, Machine L0148, Royal Master #3 centerless grinding cell:  An opening was left in the enclosure around the robot to provide access to an adjustment knob.  The opening allowed access to the working envelope of the robot.  B.	Building 22, Sphericals:  A Lectro-Etch Type 15 engraving machine was operated using a single unprotected wire located next to the point of operation.    C.	Building 22, CSP (Customer Supplied Product):  Machine L0262, a Dainichi turning center, was not provided with in interlock on the access door.  The door could be opened during the automatic mode.  D.	Building 30, Maintenance Department:  A hood guard was not provided over the chuck on Machine L0318, a Nardini horizontal lathe.  E.	Building 30, Pratt Pin Department:  Machine L0093, a Cubic GT Mini Turning Center was not provided with an interlock on the access door.  F.	Building 30, K-Flex Department:  Machine L0114, a Sunnen HTA-2100 honing machine, was not provided with safeguarding for rotating parts that were not protected by the shroud built into the machine.
