29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   (a) On or about June 24, 2024, the employer did not ensure frequent and regular inspections of the job site were conducted by a competent person to ensure hazardous conditions were found and corrected. Hazards identified included, but were not limited to fall protection, safety glasses, and hard hats not being worn.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about June 24, 2024, employees were performing clean up on the ground without a hard hat on while other employees were installing shingles on the roof above, exposing the employees to a struck-by hazard.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about June 24, 2024, employees were performing roofing work with pneumatic nail guns without the use of eye protection, exposing them to a struck by and impalement hazard.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems.   (a) On or about June 24, 2024, employees installing shingles on a commercial building with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 16 3/10 feet from roof to the ground below.
