Hazards identified included, but were not limited to fall protection not being worn and ladders not installed and/or used properly.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about August 7, 2024, at a residential building located at 8820 South Elizabeth Drive, Oak Creek, Wisconsin, employees were exposed to fall hazards in that:  (a) Employees on the upper two-story roof, conducting underlayment installation, were not protected from a fall hazard of approximately 19 feet; (b) Employees on the one-story northeast roof, moving bundles of shingles onto the ridge, were not protected from a fall hazard of approximately 9.5 feet; (c) An employee on the one-story southwest roof, conducting underlayment installation, was not protected from a fall hazard of approximately 10 feet;

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about August 7, 2024, the employer did not ensure that employees tasked with re-roofing had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall hazard on different levels of the residential property approximately between 19 to 9.5 feet to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about August 7, 2024, the employer did not ensure the divided upper portion of an extension ladder used by employees to access and egress the northeast side of the residential roof extended at least three (3) feet above the upper landing, exposing employees to a fall hazard of approximately  9.5 feet to the ground below.  (b) On or about August 7, 2024, the employer did not ensure the divided lower portion of an extension ladder used by an employee to access and egress the southwest side of the residential roof extended at least three (3) feet above the upper landing, exposing the employee to a fall hazard of approximately 10 feet to the ground below.
