This exposed employees to a fall hazard of approximately 10-feet 2-inches on the West side of the structure.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or were not secured: (a) On or about July 1, 2024, at a jobsite located at 10353 S George Dr, OAK CREEK, WI 53154, the portable extension ladders being used to access the upper landing area or roof of the structure on the West and East sides of the structure did not extend at least three feet above the upper landing area and were not secured. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately 10-feet 2-inches onto grass, building materials, and concrete .
