29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation  (a) On or about October 24 , 2024, employees were operating pneumatic nail guns without eye protection while installing shingles on the east and west side of existing residential roof, exposing employees to a struck by / eye injury hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  On or about October 24, 2024, employees conducting roofing work on a 6:12 pitch roof, on the second story of a residential home were not tied off. This exposed employees to a fall hazard of approximately 17 feet from roof to the ground below.  (b) On or about October 24, 2024, employees conducting roofing work on a 6:12 pitch roof, on the first story of a residential home were not tied off. This exposed employees to a fall hazard of approximately 9 feet from roof to the ground below.  (c) On or about October 24, 2024, employee conducting roofing work on a 6:12 pitch roof, on the first story of a residential home was not using fall protection. This exposed employees to a fall hazard of approximately 9 feet from roof to the ground below.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On or about October 24, 2024, employees installing shingles on an existing residential home, used 3 aluminum extension ladders to access the roof that were not erected three (3) feet above the upper landing.
