29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) On or about June 27, 2024, and at times prior, the employer exposed employees to caught-in and crushing hazards, in that the feed rollers on the Langston Titan rotary die cutter machine (Serial No. J-6355) were not adequately guarded, leaving a 1.25 inch opening between the feed table and the bottom of the guard where employees' hands can contact the powered feed rollers.
