(a) On or about July 1, 2024, at a jobsite located at 1357 E. Wild Ginger Way, Oak Creek, WI 53154, employees were operating hand tools to include battery powered circular saws and pneumatic nail guns without eye protection while installing the roof, to include shingles. This exposed the employees to struck-by and eye impalement hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about July 1, 2024, at a jobsite located at 1357 E. Wild Ginger Way, Oak Creek, WI 53154 , employees conducting roofing activities (shingle installation) on a 6:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the first floor roof of approximately 11 feet on the East side of the structure. (b) On or about July 1, 2024, at a jobsite located at 1357 E. Wild Ginger Way, Oak Creek, WI 53154., employees conducting roofing activities (shingle installation) on a 6:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard from the second story roof of approximately 18 feet 6 inches on the West side of the structure at the eave and in excess of 25 feet at the ridge of the roof.

Wild Ginger Way, Oak Creek, WI 53154, two of the portable extension ladders being used to access the roof of the structure did not extend at least three feet above the upper landing area nor were they secured. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately 11 feet onto grass, gravel, and building materials .
