29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about July 3, 2024, at an establishment located at 249 North Lake Street in Neenah, Wisconsin: the employer failed to ensure that a competent person onsite was performing frequent and regular inspections to identify and eliminate hazards at the construction site.  There was a Caterpillar Skid Steer (Model: 242D Serial CAT0242DADZT03952) that was not in serviceable condition due to having a broken mirror and the tires that were separating from the rim, and there was compressed gas cylinders stored outside, unsecured, and exposed to vehicular traffic, creating a projectile hazard should the cylinder fall and damage the valve.
