At the worksite, located at 5138 Madison Avenue Tampa, FL 33619; On or about 7/8/24, the employer exposed employees to amputation hazards in that machine guards were not provided for exposed rotating rollers for the Free Loop Dull Roll Machine on the production line. b.) At the worksite, located at 5138 Madison Avenue Tampa, FL 33619; On or about 7/8/24, the employer exposed employees to struck by hazards in that machine guards were not provided for the continuous movement of roof shingle material processed on the Free Loop Dull Roll Machine while on the production line.
