OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/crush-by hazards while performing auto services with defective automotive lifts: Golf Maintenance Building, Repair Shop: On and/or before July 5th, 2024, the employer did not ensure that defective Western Hoist Model WS090A was taken out of service and inspected annually by a qualified person. The left and right hand equalizer cable was not taut as well as the rear right hand swing arm assembly restrain mechanism teeth were worn on the Western Hoist vehicle lift. Employees were potentially exposed to struck-by/crushing hazards from vehicles falling from or shifting on automotive lifts.
