The employer had not conducted noise monitoring for work tasks assigned to employees which potentially exposed employees to hearing loss.

The employer had not provided a medical evaluation to determine the employee's ability to wear the multiple types of respirators assigned to be worn by employees which potentially exposed employees to hazards that could exacerbate underlying medical conditions or create upper respiratory inflammation.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Reference: ANSI Z358.1-2014: Emergency Eyewash and Shower Standard: 5.1  Performance of Eyewashes 5.1.1  A means shall be provided to ensure that a controlled flow of flushing fluid is provided to both eyes simultaneously at a velocity low enough to be non-injurious to the user. 5.4 Installation: 5.4.2 Be in accessible locations that require no more than 10 seconds to reach. The eyewash shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit its immediate use.  Golf Maintenance Shop: On and before July 10, 2024, employees conducted the following: a. Large and small scale Toro equipment pesticide, herbicide, fungicide, fertilizer application and cleaning operations and used corrosive chemicals such as Lesco Tank Cleaner and Prograss Herbicides along with a wide variety of eye hazardous chemicals, and b. Battery charging operations  The employer had not provided a properly operating eye wash station within a travel distance of 10 seconds for first aid response which potentially exposed employees to corneal and retinal burns.
