29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 727 S Newberry Street, York, PA, Roof - On or about July 11, 2024, employee's replacing a residential 6/12 pitch roof were not utilizing acceptable means or methods of fall protection, exposing employees to a fall hazard of approximately 18 feet to grade. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) 727 S Newberry Street, York, PA, Front - On or about July 11, 2024, employees were performing residential roofing work approximately 18 feet above a lower level, utilizing an extension ladder that was not extended at least three feet above the upper landing surface, exposing the employees to a fall hazard. b) 727 S Newberry Street, York, PA, Rear - On or about July 11, 2024, employees were performing residential roofing work approximately 13 feet above a lower level, utilizing an extension ladder that was not extended at least three feet above the upper landing surface, exposing the employees to a fall hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
