Located at the job site at 1905 Stelzer Road, Columbus, Ohio 43219: On or about July 11, 2024, the employer did not ensure that there were adequate cave-in protection measures in place while removing the existing storm drainage system and installing the new storm drainage system. As a result, employees were exposed to crushing hazards associated with a potential cave-in.

For each employee engaged in a task identified on Table 1, the employer shall fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section. a. Located at the job site at 1905 Stelzer Road, Columbus, Ohio: On or about July 11, 2024, the employer did not implement the engineering controls, work practices, and respiratory protection specified in Table 1 for employees exposed to respirable crystalline silica while performing tasks such as, but not limited to, cutting of a 36-inch wide concrete pipe with a masonry saw. As a result, employees were exposed to inhalation hazards.

The employer shall ensure that each employee covered by this section can demonstrate knowledge and understanding of the paragraphs (A)-(F) of 1926.1153(i)(2)(i): a. Located at the job site at 1905 Stelzer Road, Columbus, Ohio: On or about July 11, 2024, the employer did not ensure that an employee operating a concrete masonry saw could demonstrate knowledge and understanding of the health hazards associated with exposure to respirable crystalline silica. The employee was dry-cutting a 36-inch concrete pipe with a masonry saw and no respiratory protection. As a result, the employee was exposed to respirable crystalline silica.
