29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  On or about July 12, 2024, the employer did not ensure employees were protected by a protective helmet.  Employees were exposed to a possible danger of head injury from impact or from falling or flying objects, while working below employees tearing off roofing material from the second story roof of an existing residential property.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about July 12, 2024, employees conducting roofing work on an existing residential roof with an 8:12 pitch, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 20 feet over the back of the house and 10 feet over the front of the residential property.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about July 12, 2024, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work. This exposed employees to a fall hazard of approximately 20 feet over the back of the house and 10 feet over the front of the residential property.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a ) On or about July 12, 2024, the employer did not ensure the two Werner portable extension ladders used by employees to access the roof of the residential home extended at least three feet above the upper landing, exposing employees to a fall hazard of approximately 20 foot fall to the ground below.
