29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about July 23, 2024, at a jobsite located at 1150 E Prairie View Avenue, Oak Creek, WI 53154 the employer did not ensure employees were protected from flying objects by safety glasses. Employees were exposed to possible struck-by injuries from flying objects, while using a pneumatic nail gun to install shingles on the residential structure.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   (a) On or about July 23, 2024, at a jobsite located at 1150 E Prairie View Avenue, Oak Creek, WI 53154, employees conducting roofing activities (shingle installation) on a 6:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of between 10 feet 5 inches up to and in excess of  25 feet onto grass and concrete.

(a) On or about July 23, 2024, at a jobsite located at 1150 E Prairie View Avenue, Oak Creek, WI 53154, the employer did not ensure that employees exposed to potential fall hazards had been properly trained to recognize, and minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard of between 10 feet 5 inches up to and in excess of  25 feet onto grass and concrete.

(a) On or about July 23, 2024, at a jobsite located at 1150 E Prairie View Avenue, Oak Creek, WI 53154, three portable extension ladders being used to access the roof(s) of the structure did not extend at least three feet above the upper landing nor were they secured. Exposing employees to a fall hazard of approximately 10 feet 5 inches up to 19 feet 6 inches onto grass and building materials.
